Why Should You Test for Fentanyl?
Fentanyl is 50-100 times more powerful than morphine1; let that sink in for a second.

This means that even in small amounts, fentanyl can be fatal2. The problem? Most people are unaware that the drug they are taking contains fentanyl. In turn, fentanyl is a major contributor to overdose deaths in Richmond, VA, and across the country.
However, fentanyl test strips offer a simple and affordable way to detect the presence of this potent substance before consumption—potentially preventing an overdose and saving lives. So, how do they work? And most importantly, how do you use them?

How Do Drug Test Strips Work?
These thin paper strips are coated with special antibodies that react when they come into contact with fentanyl or similar substances. To use them, a small sample of the drug is mixed with water, and the strip is dipped into the liquid for about 15 seconds. Within minutes, the results appear: one line means fentanyl is present, and two lines mean it’s not. No lines at all indicate the test is invalid3.
Originally designed for urine drug tests in medical settings, these strips have been adapted so anyone can use them—no medical training needed. While they’re not perfect and may miss some fentanyl variations4, they’re highly sensitive and can detect as little as 20 nanograms per milliliter. That means they can catch amounts that could easily be deadly, making them a powerful tool for harm reduction.
Are Fentanyl Test Strips Reliable?
It’s estimated that most strips are between 96% and 99% accurate in detecting fentanyl5. The truth is that no test is 100%. Even if you receive a negative result, a drug could potentially contain very small amounts of fentanyl. There is, of course, also always room for human error when conducting the test.

How Do You Use a Test Strip for Fentanyl?
There are a few ways to do this, but the best way involves:
Step 1: Add the Drug to Water
For every 10 milligrams of meth, MDMA, or esctasy, add one teaspoon of water (about five milliliters). For other drugs, use only a ½ teaspoon of water. Crush the drug (if necessary) and mix them together.
Note: Make sure the container is clean and dry before use to avoid contamination.
Step 2: Use the Test Strip
Place the wavy side down into the water. Let the strip absorb the water for 15 seconds.
Step 3: Remove the Strip
Take it out of the water and set it on a flat surface. Leave it there for two to five minutes.
Step 4: Check Your Results
One red line means the drug is positive for fentanyl. Two lines mean it’s negative. Any other result indicates an invalid test result.
Discard the batch if it’s positive. Using it could have fatal consequences. At the same time, again, keep in mind that a negative test isn’t 100%. There is still a chance of some fentanyl or a potentially “false negative.”
Testing Tips
- Do not use the same test strip more than once.
- Make sure to crush any tablets or pills first.
- Follow the directions carefully; not doing so can result in an invalid result or false positives or negatives.

How To Get Fentanyl Test Strips
The Richmond and Henrico Health District (RHHD) offers these test strips at local Resource Centers and during outreach events. Organizations like Health Brigade, Nationz Foundation, and the Virginia Harm Reduction Coalition also provide free fentanyl test strips—no ID or insurance needed and no questions asked. Some programs even offer mail delivery for those who can’t access services in person.
